EPA Certification (Required for Refrigerant-related purchases)
Driver's License

As you complete the Application, you will need to provide us with:

Your Business FEIN
Your Personal Social Security Number

For Prompt Processing:

Ensure that your corporate Entity name is entered as Recorded with Your Secretary of State
If your business structure is a Sole Proprietorship:
- Enter your First / Last Name as the Legal Entity
- Feel free to add your Assumed Business Name under the DBA Section of the Application
